Assessment forms
The set of documents should be submitted in person at the Secretariat of SDNSP (during office hours – see Contact) or you can send scans of the documentation in PDF to the address szkola.nsip@uw.edu.pl If you send scans please include in the title of your message the following information: IPB-discipline-your name
Below you will find documents you should submit in order to pass the academic year. They should be submitted in September.
1. Individual Research Plan
The form must be submitted by 30 September 2020
2. PhD Student’s Annual report
The completed form and attachments must be submitted until the 15 September 2020.
Note: the report should include:

Supervisor’s opinion confirmed by signature

Practice form, it should be previously presented to the internship coordinator for the discipline to enter the credit in USOS

External seminar form, (ONLY in case of seminars outside the University of Warsaw or the seminars without an USOS code), before submitting it should be presented to the discipline coordinator in order to enter the credit for the research seminar in USOS
The method of submitting the forms will be determined later.
