In accordance with the recruitment schedule the following documents shall be submitted by the candidates recommended to be admitted to the School by the 22th of September 2021:

1) the original document entitling them to take up education in the School or an official copy thereof (e.g. diploma of completion of the second-cycle programme)

2) the following documents printed out from the account in the IRK system:

  • a declaration concerning undertaking education in the Doctoral School
  • an application for admission to the Doctoral School

Doctoral scholarship – statement
In order to receive the scholarship it is necessary for you to provide the secretary’s office with some data (e.g. your Polish account number). All the first year doctoral students are kindly asked to fill out the FORM [LINK] and submit the original to the secretary office.Notice! Due to the technical requirements of the UW financial system you are asked to fill out the data in the application form regarding the tax office that is proper to your address of residence. There are no tax deductions from the scholarships and you will not receive from UW the Personal Income Tax forms on this basis.ZUS (Social Insurance Institution) declarations
Doctoral student scholarships are subject to contributions. During a visit at the Doctoral Schools Secretary’s Office you will be asked to sign ZUS declarations. Doctoral students are kindly asked to check whether they are already covered by health insurance (e.g. by their parents until they are 26 years old, by their spouse or employer or as a result of receiving a pension).

Opening hours: In order to make an appointment at the Doctoral Schools Secretary’s Office and submit the documents the candidates recommended to be admitted to the School are requested to use the tool available HERE.

Note: The candidates are requested to bring with them an identity document with a photograph, a protective mask and their own pen.
