PhD student’s annual report 2021/2022

Below you will find a template of the annual report of a PhD student for the academic year 2021/2022.
The report must be submitted by 19 September 2022 at the latest.

Method / place of submitting signed documents:
– via university e-mail to the address phd.nh@uw.edu.pl.

The report should be accompanied by:
– practice credit sheet,
– an opinion that the supervisor may send directly to the school office (to the address phd.nh@uw.edu.pl) or delivered through a doctoral student.

Caution:
please enter in the title of the e-mail message in which the documents are submitted: Annual report – name and surname of the doctoral student – discipline.
