PhD student’s annual report

Please find below PhD student’s annual report forms. PhD students should submit the report until 15.09.2020.

The forms and guidance have been already sent via email to your email addresses. Please remember the document must be completed either in English or in Polish. The report should be filled out and signed by the PhD Student. The document should be scanned and sent via email to the email address of the Doctoral School of Humanities.

Notice! PhD student’s annual report should be submitted together with the opinion of the dissertation supervisor and arkusz zaliczenia praktyk.
