PhD student’s annual report 2022/2023
Below you will find a template of the annual report of a PhD student for the academic year 2022/2023.
The report must be submitted by 12 September 2023 at the latest.
Method / place of submitting signed documents:
– via university e-mail to the address phd.nh@uw.edu.pl.
The report should be accompanied by:
– practice credit sheet,
– an opinion that the supervisor may send directly to the school office (to the address phd.nh@uw.edu.pl) or delivered through a doctoral student.

Report: FILE (note: please do not fill in point 8 – the classes you took in the academic year 2022/2023 will be verified in the USOS system. Please ensure that the data in the USOS system are complete)
Practice credit sheet: FILE
Caution:
The annual report in electronic form must be legible and in .pdf format. It must contain handwritten or electronic signatures of the doctoral student and the supervisor or supervisors. If the document contains handwritten signatures, the entire document should be scanned, not just the last page with signatures.
Please enter in the title of the e-mail message in which the documents are submitted: Annual report – name and surname of the doctoral student – discipline.
