PhD student’s annual report

Please find below PhD student’s annual report forms. PhD students should submit the report until 15.09.2020. The forms and guidance have been already sent via email to your email addresses. Please remember the document must be completed either in English or in Polish. The report should be filled out and signed by the PhD Student. The document should be scanned and sent via email to the email address of the Doctoral School of Social Sciences

Notice! PhD student’s annual report should be submitted together with the opinion of the dissertation supervisor or at least the information of the the approval of the dissertation supervisor (confirmed by the signature of the supervisor or an email sent from the supervisor’s email adress registered in the domain uw.edu.pl).
